The form is designed to fit on four 8.5 by 11 inch sheets using 1 inch margins and the Courier New font with 12 point type. This worksheet is designed to ease the process of collecting the information necessary to fill out an application for NSSAR membership. The information is listed in the order it is required on the official application. The actual application must be on an official NSSAR application form and submitted through a NSSAR Chapter. If there is no local Chapter it may be submitted through the State Secretary for State at Large membership. For the location of the nearest Chapter write to NSSAR, 1000 Fourth Street, Louisville, KY 40203. ----------------------------------------- INSTRUCTIONS: Line 1 Enter the full name of the Patriot Ancestor. The Patriot Ancestor must be direct bloodline i.e. without adoptions. For Gen# count yourself as #1. Line 2 Enter the Patriotic Service of the Ancestor. This should include Rank and State of Service for those who served in the Military; Patriotic Service (enter PS) for those who signed a declarations, rendered aid, served as an elected official, etc. Lines 3-6 The FULL name of the person making application followed by his address and telephone number. Social Security number is optional. Lines 8-13 The Names of children. Enter wife number if more than one spouse. All dates on this application should be in the format 4 Jul 1776 (DD MMM YYYY). Lines 18-26 Enter the applicant’s name, date of birth and Place (location) of birth. All locations should be in the format City/County/State. If one is not available or applicable enter the slash (/) to show positional value. If it is a non-US or Canadian location enter it in normal form for that place. Enter date and location of marriage(s). If only one marriage leave second blank. If more than two marriages attach a sheet listing those which resulted in children (i.e. provide the mother’s of all children listed in lines 9-13). Lines 27-103 Enter the appropriate information for each generation through the Patriot Ancestor. Do not enter any information for generations before the Patriot Ancestor. Line 104 Enter the Cemetery and location of the cemetery where the Patriot Ancestor is buried. This is an optional field. Lines 106-115 Enter the proof of lineage. For the first three generations "primary" evidence must be supplied. Primary evidence includes birth certificates (baptismal certificates may be substituted if no birth certificate is available), marriage certificates, death certificates, etc. The lineage and date for each event must be proved by this means. It is not necessary to furnish each document if evidence is found on another document. For instance a death certificate may show a parent’s birth, death, parent’s, marriage, spouse name, etc. Lines 116-142 Enter proof of lineage. For the fourth and later generations secondary evidence may be use. Typical secondary evidence would include published books (typewritten or computer- generated books are not acceptable), Census records, cemetery records, etc. Attach a photocopy of the material cited; photographs of tombstones may also be used. Note that references to previous SAR, DAR or SR applications for the same lineage may be cited provided that such application had the appropriate acceptable proof. Actual copies of SAR applications are not required. For DAR or SR applications cited a RECORD COPY of such application must be included. Lines 143-146 References to a Patriot Ancestor’s War Service MUST be included. specific references to service. The references may be either primary (copy of a Pension application for example) or secondary. Line 147 Applicant’s occupation.
